
Braden Scale
The Braden Scale is a clinical tool used by healthcare providers to assess a patient's risk of developing pressure ulcers, also known as bedsores. It evaluates six areas: sensory perception, moisture, activity level, mobility, nutrition, and friction/shear. Each area is scored to reflect how vulnerable a person might be to skin breakdown due to prolonged pressure, immobility, or poor skin condition. The total score helps caregivers identify those at higher risk and plan preventive measures, ensuring timely interventions to maintain skin integrity and patient comfort.
