The Demographic Transition Model describes how cities and countries change over time in terms of birth and death rates. It includes five stages: 1. **Pre-Transition**: High birth and death rates, slow population growth. 2. **Transition**: Death rates drop due to improvements in healthcare, leading to rapid population growth. 3. **Mature Transition**: Birth rates begin to decline, stabilizing population growth. 4. **Post-Transition**: Low birth and death rates, leading to a stable or slowly growing population. 5. **Declining**: Birth rates fall below death rates, resulting in population decline. This pattern reflects economic and social development levels.
