
Transition Services directive
Transition Services are a set of coordinated activities designed to help students with disabilities move from high school to post-secondary life, whether that’s college, work, or independent living. These services are planned starting at age 16 and may include skill development, job training, counseling, and community connections. The goal is to ensure students gain the necessary skills and support to successfully transition into adulthood and achieve their personal goals. Essentially, they serve as a bridge, helping students prepare for life after high school with tailored resources and planning.
