plan types (PPO, HMO, etc.)

Common health insurance plan types include PPO (Preferred Provider Organization), HMO (Health Maintenance Organization), POS (Point of Service), and EPO (Exclusive Provider Organization). PPO plans offer flexibility to see any doctor, including out-of-network, often with higher costs. HMO plans require you to choose a primary care doctor and get referrals to see specialists, typically limiting you to in-network providers. POS plans combine features: you select a primary doctor but can see out-of-network providers with higher expenses. EPO plans cover only in-network providers, except in emergencies. Each plan balances cost, flexibility, and provider options differently to meet varied healthcare needs.
