Lauda Air Flight 004 was a commercial flight that crashed in 1991 due to a serious mechanical failure involving the aircraft's thrust reverser systems. During the flight, the reverse thrust mechanism deployed unexpectedly in mid-air, causing the plane to become uncontrollable and resulting in an in-flight breakup. All 223 passengers and crew aboard were killed.
