Frailty Phenotype Model

The Frailty Phenotype Model is a way to identify older adults who are at higher risk of health problems. It looks for five specific signs: unintentional weight loss, feeling unusually tired, weak grip strength, slow walking speed, and low physical activity. If a person has three or more of these signs, they are considered frail, meaning they may be more vulnerable to illness, disability, or recovery difficulties. This model helps healthcare professionals recognize and address issues early to improve health outcomes and quality of life for older individuals.
