CAPABLE Model

The CAPABLE (Community Aging in Place—Advancing Better Living for Elders) model is a program designed to help older adults live independently and safely in their homes. It combines a team of professionals—such as nurses, occupational therapists, and contractors—to assess an individual's needs and then provide support like home modifications, health education, and personalized strategies. The goal is to address barriers to daily living, improve overall well-being, and enable seniors to stay in their familiar environment comfortably and safely for as long as possible.
