C-CHANGE is a framework used to help caregivers and healthcare providers recognize and address key health factors that influence a person's well-being. It highlights seven critical areas: Condition (medical issues), Change (significant life events), Health literacy, Access to healthcare, Network (social support), Goals (personal health objectives), and Environment (living conditions). By systematically considering these areas, caregivers can develop comprehensive care plans, improve communication, and support better health outcomes. Essentially, C-CHANGE ensures a person’s physical, emotional, and social needs are all accounted for in their healthcare management.
