Aging life care professionals

Aging life care professionals, also known as geriatric care managers, are trained experts who help older adults and their families navigate the challenges of aging. They assess health, safety, and personal needs, coordinate medical and social services, and develop customized care plans. Their goal is to promote independence, improve quality of life, and ensure safe, respectful care. These professionals may also assist with transitions such as moving to assisted living or managing chronic conditions.
